I, Lalramzauva, S/o H.Vanramhluna, R/o Vengchhak, Taitesena Section, Near PCI, Ngopa, Saitual District, Mizoram, aged about 37 years do hereby solemnly affirm and state as under:
1. That I am bonafide citizen of India belonging to the Scheduled Tribe community of Mizo.
2. That my true and correct name is Lalramzauva and my father’s true and correct name is H.Vanramhluna which are correctly entered and recorded in my Adhaar Card and other official documents.
3. That in my Permanent Account NUMBER (PAN Card) No. ABIPZ0378H, my name has been inadvertently written and recorded as Lalramzauba S/o Hemar Vanramhluna which is incorrect.
4. That the letter ‘H’ which appears in my father’s name stands for ‘Hemar’ which is his surname.
5. That the purpose of this affidavit is to prove and declare that my true and correct name is Lalramzauva, S/o H.Vanramhluna and that it should be recorded as such in my PAN Card.
6. That the statement made in para No. 1-5 are true and correct to the best of my knowledge and belief and nothing material has been concealed therein.
