By this deed I, the undersigned Tessa Lalremruati D/o Arsimawia, a permanent resident of
North Lungleng, Aizawl District, Aizawl, Mizoram solemnly deciare:-
1. That I wholly renounce/relinquish and abandon the use of my former name Lalremruati in all
my documents i.e, Lalremruati D/o Arsimawia and in place thereof, I do hereby assume from
this date the name Tessa Lalremruati as my name so that I may hereafter be called, known and
distinguished not by my former’s name but assumed name of Tessa Lalremruati.
2. That for the purpose of evidencing such my determination dedare that I shall at all times hereafter
in all records, deeds and writings and in all proceedings, dealings and transactions, private as
well as upon all occasions whatsoever, use and sign the name of Tessa Lalremruati as my name
in place and in substitution of my former name
3. That I expressly authorize and request all persons in general and relatives and friends in particular
at  all  times  hereafter  to  designate  and  address  me  using  such  my  assumed  name  of  Tessa
Lalremruati.
4. That the statements made in para 1 - 3 are true and correct to the best of my knowledge and firm
belief.

C. Laldinchami in her Service Book and in all her other service records which should be followed by
publication of the change of name in prominent local newspapers as well as in the Mizoram Gazette at
